Indeed the "bread" is low in carbohydrates. However, the peanut butter and bananas are high in carbohydrates (20g of carbohydrates per 100g of peanut butter and 27g of carbohydrates for a typical banana).
Maintaining a ketogenic diet typically requires consuming less than 20g of carbohydrates so this sandwich will quickly put you on the path to a glucose catabolism.
