He is a smoker, but the top photo is neither current nor honest. The original was taken in 2004 when Obama was campaigning for his Illinois Senate seat.
Someone has reversed the true image (perhaps hoping that automated photo searching software wouldn't reveal the original), and added the cigarette. Now the Arizona Republican party is using this photo as part of their campaign opposing Obama's health care reform proposals.
I have definitely seen that a few different places implying it was real. Its a good photoshop which I guess is a like saying a good lie.
