The +2 annotation on his foot means his dorsalis pedis pulse is intact and the viability of the foot is not in imminent danger. The discussion thread at the link includes suggestions that perhaps this image is Photoshopped. No way (and no need to); anyone who has worked in ERs or the military has seen this kind of stuff or worse. It is, however, quite impressive...
